Yes. SAIL Home has two Community Builders who live on-site and a Community Manager who serves as the liaison between staff, service providers, residents, and families. This staff will be used to supplement support services of residents where needed. A Community Builder or the Community Manager will always be onsite.

We will schedule a brief interview with you to learn more about your loved one. After that, we will send you the Life Skills Assessment tool, which helps us determine the necessary support for potential residency at SAIL Home.

WHO CONDUCTS THE LIFE SKILLS ASSESSMENT?

The assessment is completed by the family, a current service provider for your loved one, and an independent professional assessor.

IS THERE A COST FOR THE ASSESSMENT?

Yes. There is a $500 fee to have the assessment done. This fee is non-refundable. If your loved one is not ultimately accepted as a resident, this fee will be waived for future applications to SAIL Home properties.

No. Residents must have support services in place before moving in. SAIL Home will assist families on what services they may be eligible for and how to advocate for those services. SAIL Home and families must agree on an appropriate support model for your loved one before residency. Families may have private pay for support as needed.

WHAT DOES THE COMMUNITY BUILDER DO ?

Community Builders will be available for overnight on-call support and daytime/weekend hours. Daytime and weekend hours will be determined by where there is a need for support over and above what residents bring in their service plans.There will be no support staffing available Monday–Friday between 8:30 AM and 3:30 PM, as residents are expected to be at their day programs or jobs. SAIL Home will provide the schedule of the Community Builders and Community Manager's when we are closer to residency.

WHAT IS THE ROLE OF THE PARENTS?

Parents play a key role in the success of SAIL Home. One primary responsibility is to serve as a rotating weekend volunteer once every eight weeks. Depending on weekend staffing coverage, we may request additional support from families to ensure safety and support for the residents. You should plan for 6-8 hours over the weekend to be on premises.

Emergencies at SAIL Home.

Resident’s support staff being unavailable due to illness or time off.

WHAT IF I CANNOT FULFILL MY RESPONSIBILITIES AS A VOLUNTEER OR CAREGIVER ?
